Adding the Certificate to Your Degree Program

Procedure for Addition of QIOM CERTIFICATE to Health Systems Management Degree Program:

  1. Students must first fill in the Declaration of Certificate Program portion of the “Change/Declaration of Academic Program” form. Once the form is completed by the student, it is submitted to Adam McCutcheon in Northeast Module 102 and then brought to the Office of the Registrar for processing.

  2. All Health Systems Management students wishing to earn the Quality Improvement and Outcomes Management Certificate in addition to their graduate degree must take HAP 730: Health Care Decision Analysis and ask for substitution of HAP 715 Health Economics for the elective in the Certificate.
